SLEBC Open Enrollment for the 2025 plan year will be held from November 17th to November 25th, 2024.

Elected changes will become effective January 1^st, 2025. 

If you wish to make changes, please submit the necessary forms for Medical, Dental, and Vision coverage - if you do not have changes, you do NOT need to submit forms except for Flexible Spending. If you wish to enroll in Flexible Spending for 2025, you must submit your 2025 Flexible Spending election. FORM LINK

The SLEBC Board is happy to report that we have been able to maintain the same coverages we are accustomed to with an addition of a few items. We are thankful that we do not have any major plan design changes to our medical, dental or vision plans. You will find slight increases in the medical rates due to the increasing costs seen with current inflation along with plan usage.
As a self-funded health plan, this is a direct reflection of you being mindful of your utilization and using the tools available to make good healthcare decisions.  We encourage you (and your family) to check in with your primary care provider for your annual preventative visit and continue to work toward being the healthiest you!

2025 SLEBC Benefit Changes

• Prescription Benefit Management in 2025 will be ServeYouRx and the formulary should cause very little disruption (less than 10 scripts).

• Nutrition Counseling is now available in-network for a $15 copay with a licensed dietitian or nutritionist.

• Out-of-Network Hearing Aid benefit will mirror In-Network benefits.

• GLP-1 option for weight loss is available at a discounted rate through a reputable pharmacy - this is funded by the member directly.

• There are no changes for dental, vision or additional life and AD&D premiums. There is an increase in your medical premium.

No ID Card - VSP (Vision)  Providers use your DOB and/or the last four of your social security number.

If you have a new mailing address and have not notified me, please respond to this email with your address immediately so that we can ensure your insurance documents are sent to the correct address.

Please always check your pay stubs for accuracy. If you find a discrepancy, please let me know if it is a SLEBC deduction/withholding, and I'll work to get it corrected.
