This email only pertains to those individuals who are utilizing SLEBC for MEDICAL insurance. 

This notice is to inform you of an important update to your health plan. If applicable, please share this information with your spouse and/or any older children this may impact.

Effective May 1, 2025, our health plan will transition to using Midlands Choice Premier as the network for accessing providers and facilities in our area. This will replace Aetna Open Choice PPO as the network. 

This change is necessary because CHI Health System and Aetna have been unable to come to an agreement. By moving to the Midlands Choice Premier network, we are ensuring continued access to high-quality care and a wide range of healthcare providers. 

If you have any questions or need assistance in finding in-network providers, please contact your Apta Care Coordinators at (866) 274-9478 or visit https://www.midlandschoice.com/providersearch/. (Choose Meritain Health as the Insurance Company - and the Network will be with Mayo Clinic)

I highly recommend reviewing your current providers to ensure that they are still in-network before your next appointment. The disruption that our members will have is very small - especially in comparison to the CHI provider and facility gaps - this means that you should be able to find your current providers on the in-network list!

To ensure seamless access to care, you will receive a new ID card with updated network information. Please use this new card for any healthcare services received on or after May 1, 2025. You may access a digital copy of your ID card at any time on or after May 1st by visiting SLEBC.MyAptaHealth.com.
