If you are enrolled in SLEBC Medical Insurance, please see the attached temporary ID card with new Rx information that you should use until the new card arrives.

Members who need medical care may continue to use their current ID card until the new card arrives, BUT pharmacies will need the Rx information listed on the attached temp card. If you are new to the plan and have not yet received the ID card, please feel free to email me for a digital copy effective 1/1/25.
